Dr. Foshagen is typically available for patients M, Tu and Th. She is not offering primary care services at this time. This is a consultative medical practice focused on examining root causes of hormonal and metabolic dysfunction. We offer an in-depth and personalized care package for our patients. We ask that our patients also have a primary care doctor for all urgent and some routine needs.

Yes! We offer visits by video, phone, or in the office. For in-office appointments, you may be asked to fill out a COVID-19 questionnaire before your appointment. We ask that you reschedule your appointment or switch to a virtual visit if you are ill or experiencing any symptoms such as fever, cough, shortness of breath, etc.

Dr. Foshagen does see patients who are outside of our local area, as long as they are established with a primary care physician in their area. Consultations and laboratory services are offered to these patients through remote visits, however the initial consult with Dr. Foshagen is encouraged to be an in-person consultation.

We offer a number of lab order referrals with cutting edge labs around the country. Traditional laboratory analysis is obtained through Quest Diagnostics, LabCorp, or Valley Clinical Labs. Other testing labs frequently used include Genova Diagnostics, Boston Heart Laboratories, Doctor’s Data, SpectraCell, ZRT laboratories, and DUTCH Laboratories. Click on the links below to find out more about each. Some tests are done through saliva samples, and others are done through blood draws or urine samples. Common tests include testing for gastrointestinal, cardiometabolic, and hormone health. You can click the links below for more information about testing available through each laboratory.

For patients out of the area, we can arrange for mobile phlebotomy for laboratory testing.

In order to provide this level of service, St Moritz no longer contracts with any health insurance carrier. Payment is expected at the time of service. Charges are based on time, complexity, and whether referrals or testing is ordered. Phone and telemedicine visits are available and generally incur the same charge as in-office visits. For patients with a private medical insurance plan or a Health Savings Account, they may submit our superbill and codes for reimbursement. We provide codes for all laboratory and imaging tests.

PPO Plans

At. St. Moritz Medical Center, we are considered an “Out-of-Network” provider for PPO plans. What this means for you is that the cost of visits will be paid by you at the time of your visit. You will then be provided with a superbill to submit to your insurance for possible reimbursement. How much reimbursement you get will depend on your specific plan and coverage, Please call the member services number on the back of your insurance card for information on your individual plan.

HMO, Medicare, Medi-Cal

You will not be eligible to submit to receive any reimbursement for office visits. Referrals to labs, imaging, etc. that come from these office visits may be covered through your insurance. PATIENTS WILL NOT BE ALLOWED TO SUBMIT BILLS TO MEDICARE. We cannot offer care under a patient's MediCal contract. Please call the member services number on the back of your insurance card for specifics on your individual plan.

What is the cost of a visit with Dr. Foshagen?

Medical Visits

The Initial Consult at our practice is what a patient will first come in for. It is a 60-90 minute appointment and the cost is $375. During this visit, a detailed review of medical history, a focused physical exam, and a treatment plan will be developed. Follow-ups typically range from $125-$275. Charges are based on time, complexity, and whether referrals or testing is ordered.

Aesthetic Visits

Cost depends on services rendered. Skin consultations are free for new patients. Botox and dermal filler appointments depend upon on the amount of product used and time considerations. Call the office for estimates for specific products you are interested in.

Once we have you added to as a new patient, you will fill out an online Health History Questionnaire. Dr. Foshagen will review this document before your first appointment. Your first appt will be an Initial Consult, which is a 60-90 minute appointment in the office.

Due to COVID-19, we are still operating on reduced hours. We are open Mondays, Tuesdays, and Thursdays from 9am-12pm and 1pm-5pm. Wednesdays, Fridays, and weekends we are available by appointment only. Because we may be seeing patients remotely all day during our normal hours, the office may be physically closed on some of these days. However, we will still be available by phone. If you would like to come by to pick-up supplements or paperwork, please call the office to ensure we will be physically in the office at that time.
